Candidates must reside within NorthLakes' service area at the time of hire and maintain residency throughout employment. NLCC Triage RN will provide triage and health advice by phone to patients, utilizing evidence based triage protocols, as well as processing prescription refill requests received from pharmacies and patients. Performs nursing assessment and triage. Follows clinical guidelines, protocols and policies related to direct patient care.

Complete refill requests for non-controlled substances per NLCC protocol and guidelines

Complete refill requests for controlled substances under direction and approval from prescribing provider

Promote timely access to appropriate care to reduce emergency room utilization and hospital readmissions

Connect with patients for ED and hospitalization follow-up

Provide medication reconciliation
